What is Support Coordination and what you can expect from us?
Assisting the participant to connect with support services. This may also involve development of an intervention plan which will be put in place by disability support workers. These supports focus on strengthening the participants’ ability to coordinate their support and to assist them to live at home and participate in their community.

Our services provided and charges are in accordance with the current NDIS pricing arrangements and price limits. Australian Family Support charges an establishment fee, in line with NDIS guidelines, for all new participants.
