What is Support Coordination and what you can expect from us?

Assisting the participant to connect with support services. This may also involve development of an intervention plan which will be put in place by disability support workers. These supports focus on strengthening the participants’ ability to coordinate their support and to assist them to live at home and participate in their community.

•Assistance with accommodation and tenancy obligations

We will work alongside our Nurses to ensure quality health services by working with our primary healthcare and other external providers.

Important information

Our services provided and charges are in accordance with the  current NDIS pricing arrangements and price limits. Australian Family Support charges an establishment fee, in line with NDIS guidelines, for all new participants.

You can manage your plan yourself, or have a nominee or plan manager to assist you. If it gets too complicated, consider having a support coordinator to assist you to to manage your supports.

  • Helps you understand you plans and funds
  • Connects you with service providers
  • Assist with booking and managing services
  • Helps you navigate the NDIS system

You can request support coordination as part of your NDIS plan.

Yes, you can choose your own support coordinator or have one assigned to you.

Support coordination is an ongoing service that continues as long as you have an NDIS plan that allocates the specific hours that the support coordinator can assist you.
