Australian Family Support bridges the gap between the National Disability Insurance Scheme (NDIS) services and general practitioner support for NDIS participants. We aim to increase the access of participants with NDIS plans to quality health services by integrating our support services with primary healthcare.

General Practitioner Support

At Australian Family Support, we don’t just provide regulated NDIS support services. We connect our NDIS participants with access to expert clinical support and facilities for primary health care.

About Australian Family Support

Australian Family Support was founded by two general practitioners with a vision to connect disability support requirements with access to GP services. By strengthening and expanding your access to integrated services, we can comprehensively cover all aspects of your goals, health, employment, educational and social support needs.

Everyone has questions about NDIS support. Here are answers to some of the most frequently asked questions. If you have other questions, please contact us. We are here to assist.

You may eligible for the NDIS if you answer yes to all or some of these questions:

 

  • Are you aged between 7 and 65?
  • Do you live in Australia and have Australian residency?
  • Do you usually need support from a person because of a permanent and significant disability?
  • Do you use special equipment because of a permanent and significant disability?
  • Do you need some supports now to reduce your future needs?

If you think you are eligible or would like to evaluate your options, please get in touch with us. We will connect with your GP to retrieve relevant medical information to assess. We can also connect you to your Local Area Coordinator (LAC) for assistance with disability support or make an application if you are eligible. Alternatively, you may contact NDIS directly on 1800 800110.

 Support coordination can be included in your NDIS plan. To access this service, inform NDIS that you require some assistance with your plan during your plan development. Where reasonable and necessary, support coordination will be included in the Capacity Building budget.

We can help you to connect to your local area coordinator or the NDIA to request for a plan reassessment, including for when your situation or details have changed. We will support you throughout your journey and assist you with completing your forms.

During our onboarding session with you, our staff will get to know you and learn your preferences and needs. We will try to match our support workers to you. Our friendly team will hold your hand through this journey to coordinate and provide the relevant services, catering to your individual needs. However, if you are not happy with the selection of support workers, we will try our best to find a suitable candidate to your preferences. You have the choice and control.

AFS is led by family Doctors and clinicians with many years of providing health care. We understand the complexities of health care and support delivery and the impact of illness and disability on the whole family. Our sister company, Australian Family Care provides easy access to General Practitioners, via our two clinics:

Our support workers all have relevant vocational qualifications and are supported to continue their skills development. Team members have First Aid and CPR certificates and current Working with Children Check, Police Check, NDIS worker screening check, current driver’s license and access to a vehicle. 
Our support workers also receive ongoing in-house training as part of our commitment to providing high quality, person-centered, best in class services to our participants and their families. Our commitment and dedication to continuously improving ourselves and the quality of service delivery is always assured.

Where you provide AFS with less than four (4) business days’ notice you will be charged 100% of the service fee that was scheduled in your roster. 

A cancellation is considered to be a short notice cancellation if the participant:

  1. Does not show up for a schedule support within a reasonable time, or is not present at the agreed place and within a reasonable time when the provider/support worker is travelling to deliver the support; or
  2. Has given less than 4 clear business days’ notice for a support that meets both of the following conditions
  3. The support is less than 8 hours continuous duration; AND
  4. The agreed total price for the support is less than $1,000.00

AFS will only claim from a participants plan for a short notice cancellation of the delivery of a support item to a participant if all of the following conditions are met:

  1. The provider has the agreement of the participant in advance by way of this Service Agreement being signed by the participant or documented Advocate
  2. The provider was unable to find alternative billable work for the relevant worker and are required to pay the worker for the time that would have been spent providing the support.
  3. Claims for a short notice cancellation will be made using the same support item as would have been used if the support had been delivered, using the “Cancellation” option in the MyPlace Portal.

 

There are no set limits on the number of short cancellations (or ‘no shows’) for which AFS can claim in respect to each participant. However, AFS have a duty of care to their participants and, where a participant has an unusual number of cancellations, then we would seek to understand why this is  occurring.

The NDIA monitor claims for cancellations and may contact AFS who have a participant with an unusual number of cancellations.

If you wish to exit AFS NDIS service to move to another provider, we request that you  provide AFS with 2 weeks’ notice in writing. We can help connect you with another provider if you like.

There are no fees to exit the services of AFS, unless you provide less than 7 business days’ notice to exit our services, where the Short Notice Cancellation Policy will be applied.

Yes, you can change your service provider at any time, but you may need to provide notice as per the service agreement that you have signed and agreed upon. 

You can review your service agreement, provide written notice (if required) and discuss the transition with them.

AFS will try to minimise disruptions by coordinating with both providers.

AFS hires support worker with relevant vocational qualifications. Rest assured that our staff have up to date First Aid and CPR certificate, Working with Children Check, Police Check, NDIS worker screening check, current driver’s license and access to a vehicle. Our support worker are also trained to perform relevant skills. AFS aims to continue to provide high-quality, person-centered, best in class, services to our participants and their families.
